Our risk assessment form isn't fancy or decorative - just a regular spreadsheet for the sake of formality. Generally speaking, since we were shooting at a new location, there were a lot of risks as mentioned in the sheet below. It has all the technicalities of filming, which we only recently learnt about and really understood. Clearly, shooting a film project isn't just fun and games, so here it is (bland and dry, yes, but definitely needed):
